  • Evaluation & Referral

    Every journey begins with a Face-to-Face evaluation between the patient and their Medical Provider. If mobility equipment is deemed necessary, the physician will send a referral directly to Momentum Mobility Inc. This referral ensures our team has the clinical direction needed to start the process and collaborate with your care team effectively

  • Medical Documentation

    Once the referral is received, our team works closely with your Physician and Therapist to collect all required medical documentation. This includes mobility evaluations, therapy notes, and supporting medical records. We make the process seamless — ensuring everything meets funding and insurance requirements while saving you time and stress.

  • Authorization & Insurance Approval

    With documentation in hand, Momentum Mobility Inc. submits the full packet to your insurance provider or funding source —I.e Medicare, Medicaid, or private insurances. They’ll review the medical necessity of the equipment.
    If a denial occurs, we don’t stop there — our team will follow up with your doctors to gather additional documentation and resubmit the request, working tirelessly until your approval is secured.

  • Delivery & Final Fitting

    Once Prior Auth is approved and the product is manufactured, your equipment is delivered and fitted by a certified Assistive Technology Professional (ATP) and technician. They’ll walk you through setup, ensure proper fit and comfort, and provide hands-on training for operation, maintenance, and any custom features. We make sure your equipment is ready to support your lifestyle from day one.
